Understanding comes from knowing. Caregiving can be quite challenging. “Thriving Under Pressure” is a
three-part webinar series that will explore the many facets of caregiving. Part Two on May 23 will discuss resources available to assist caregivers of those being discharged from the hospital to another care setting and those who are already in the community and in need of care and information.
Learning Objectives
• Learn about resources available to caregivers of those preparing to leave the hospital.
• Learn about community-based caregiver resources.
• Learn how caregivers can advocate for their loved ones and for themselves.

Meet Our Panelists
Zackary Brooks, LCSW, ACSW
Zackary Brooks is systemwide Director of the Transitions of Care Department for Cone Health. He serves patients and their family members by identifying what is needed for discharge planning, including home health, durable medical equipment, rehab placement, and provision of resources for patients with Primary Care Physician follow-up. He started his career in social work 29 years ago after receiving dual undergraduate degrees in Social Work and Political Science from UNC Greensboro. He received his graduate degree in Social Work (MSW) from UNC Chapel Hill.
Geronda Pulliam, RN, BSN, MS, CCM
Geronda Pulliam is Assistant Clinical Director of Ambulatory Care Management at Triad Healthcare Network. Geronda has 33 years of nursing experience, including case management in community and acute care. She received her Bachelors of Science degree in Nursing and Masters of Science degree in Gerontology from the University of North Carolina at Greensboro, and a Post Graduate Certificate in Nursing Administration from the University of North Carolina at Charlotte. She holds a national Certification in Case Management (CCM) and also has personal experience as a caregiver.
Meet Our Moderator
Risa Hanau, LCSW, is the Clinical and Community Educator at AuthoraCare Collective. Risa is a seasoned presenter and teacher in the area of end-of-life and ethics. She is a member of the Cone Health Ethics Committee. She is a Licensed Clinical Social Worker by training and holds a Master of Social Work from The University of Pennsylvania.
