Most advanced illness or palliative care is provided in one’s own home. Your AuthoraCare team will schedule regular visits to the patient’s home. They will coordinate a treatment plan with the patient’s regular doctors to ensure that all care is aligning with the patient’s and family’s health care wishes.
Advanced Illness Care in Long-Term Care Communities
AuthoraCare also serves patients who are currently residing in skilled nursing, memory care, assisted-living, independent-living and group home communities. Our team works closely with facility staff members to serve residents with a life-limiting illnesses. The frequency of visits is based on the needs of the patient.
